Your sponsoring firm must
file an application form (Form U4), along with the appropriate
testing and registration fees (both federal and state) on your
behalf.
Upon receipt of your application and fees, the NASD will send your sponsoring
firm an enrollment confirmation, which will be valid for 90 days. You will
be required to schedule your examination within that 90-day period. |
Unsponsored Securities
Exams

Certain securities licenses
do not require prior authorization from NASD. In other words, affiliation
with an NASD member firm is not necessary. Unsponsored exams include
the Series 3, Series 30, Series 31, Series 63 and Series 65. If
such is the case with the license you seek, then the responsibility
for proper exam registration and scheduling lies with you. In order
to register, you must submit Form U-10 (Uniform Examination Request
for Non-NASD Candidates) along with the apporpriate testing fee
to the NASD. |

NASD examinations are administered
on the PROCTOR computerized testing system. PROCTOR is a nationwide,
interactive computer system designed for the administration and
delivery of qualification examinations.
When you have completed the exam, the PROCTOR System promptly scores your
answers and within minutes displays a grade for the exam on the terminal
screen. |

NASD Registration Application

NASD Fingerprint Processing Fee - $24.50
NASD SPR Fee - $95.00
This fee covers the processing of Form U-4 filings that require special
Registration Review due to either a yes answer on pg. 3 of the U-4 or disciplinary
info. in the individual's CRD record.

Take one form of personal
identification that bears your signature and your photograph as
issued by a government agency. You are not allowed to take reference
materials or anything else into the testing area. Calculators will
be available upon request; you will not be allowed to use your
personal calculator.
Scratch paper and pencils will be provided by the testing center. |

In order to take an examination
you must first become enrolled on the PROCTOR System. Your firm
must have filed an application and an examination fee with the
NASD. In return you will receive an PROCTOR enrollment or confirmation
of enrollment, along with a listing of current PROCTOR test centers.
This confirmation of enrollment has an expiration date, and you
must take your examination prior to that date.
You cannot schedule an appointment if you are not enrolled, and you cannot
schedule a test beyond the 90-day enrollment period. The staff of a test
center cannot help you if your enrollment does not exist or it has expired.
You will need to reapply to the NASD by filing another application and
another testing fee before you can be re-enrolled on the system.

NASD prohibits making extensions
on examination. If you have made an appointment and you find that
you cannot make the test date scheduled, it is imperative that
you cancel your appointment with the test center properly, or you
may incur additional fees. Under NASD guidelines, an appoint cancellation
will be considered timely if effected according to the following
schedules: |

If you follow the guidelines
provided, you may cancel or reschedule your examination without
penalty. If you do not, the NASD will charge member-firms a cancellation
fee equal to the amount of the examination fee. The same cancellation
fee will be charged if you do not show up for an appointment, or
if you arrive too late to take the examination. You will be considered
too late if your arrival and subsequent testing will upset the
schedule for the test center for that date. |

The examinations are given
in over fifty locations around the country in what are called Professional
Development Centers. These centers, which are normally open from
Monday through Friday, only administer the examinations based upon
information received from the NASD; therefore, if you encounter
problems at a testing center, you will usually have to resolve
them with the NASD.
